Well we have all seen at least one of these incidents on site I should think, and they certainly make a good advert for the modern ejector systems that are gaining popularity.
Here at no 5, This Volvo A35C is looking in a very bad way after attempting to go under this bridge with the body still up!

These pics reminded me of my ADT adventure , to start the story, setting Mcdowell county WV., job-building mine ponds 200ft by 300ft and 50ft deep .
I was driving 1 of 2 of our A35C Volvos on May 16th 2009 , the main part of the pond was done and we were cleaning up and making final cuts with a komatsu Pc300-7.
If you can picture this in your head the haul road was up a slight grade to the dump site and I was coming down from dumpin the previous load in the slate dump (dump site) and well the road forked and you had to back out on the 50ft highwall of the pond to get loaded and i was backing up to get loaded and the truck tilted slightly (not knowing I was right on the edge of the wall) I thought it was a rock or a rut from the other truck and I cut the wheel sharp to the right therefore making the left side of the truck bow out to the left (mostly the cab) and it tilted more then I got a little scared and realized I was about to roll off the wall and face death I put the truck in forward gear and the fuel to the floor and by then I was making the first slow roll and in the process.
I was screaming every word I could think of with four letters at the top of my lungs, then rolling 4 more times and got flung around the cab like a dogs chew toy and covered with dirt and glass and bleeding from some minor cuts on my back and head , it all stoped and like in the movies you lay their for about 2 seconds thinking your dead after that I crawled out of the windshield area since the truck was on its side and over to the bank and set half way in the fetal postion and by then the hoe operater and 2 other truck drivers were runing down the sloped part of the pond being cut towards me screaming are you ok horsie (my nick name) .
The job was shut down for the rest of the day .
I cheated death for a small price-a bruised rib and 5 mid size cuts .
I was not wearing a seat belt or hard hat . I could of easly been killed that day but the good lord was watching over me and my fellow co-workers that day . I do not have pics of the accident .
